TERMS OF REFERENCE FOR Development of the Clinical Management of Rape Guidelines/ Protocol for South Sudan Hiring Office: UNFPA South Sudan Country Office Purpose of consultancy: Gender-based violence, including rape, is an acknowledged problem throughout the World. If left unaddressed, Sexual Violence may have serious negative personal and social consequences for the survivors as well for their families and the larger community. It is critical to prevent Sexual Violence because is a Human rights violation. Survivors may suffer from depression, anxiety, contract HIV or other STIs, un-wanted pregnancies or stigma from family and community members. Historically Sexual Violence has consistently arisen in high proportion result of situations of conflict, forced migration. It continues to be so today.
The recent instability and conflict has also exacerbated the incidence of rape cases in South Sudan. According to the GBV Strategy (2017), rape is a common feature of the conflict threatening civilians inside and outside of the Protection of Civilians (POC) sites.1 Gang rapes and abductions of women and girls by armed actors are reported regularly, often occurring when civilians cross military checkpoints; flee areas under military attack or when they leave PoC sites to collect firewood or food.2 This has resulted into increased need for Clinical Management of Rape Survivors.
Given the immediate as well as long-term negative effects of rape and sexual assaults on the physical and mental health of survivors, medical response as a livesaving measure is very critical.
Clinical management of rape (CMR) is therefore a fundamental component of the comprehensive response needed by survivors of sexual violence and one of the most critical and life-saving responses, hence requiring systematic, streamlined and well-coordinated efforts to strengthen ongoing CMR initiatives by all the stakeholders in South Sudan
Since 2014, a series of trainings on CMR/PFA have been undertaken to build capacity of Health workers in this domain. The training materials used is based on IRC Clinical Care for Sexual Assault and WHO Clinical Management of Rape Survivors’ protocols. Over 400 Health workers have been trained on CMR. Currently over 65 Health facilities have capacity to respond to Sexual Violence cases with support from UNFPA. The assessment conducted in September 2016 revealed that 30 out of 35 health facilities reported that they are providing CMR/PFA services in humanitarian settings. Under the leadership of UNFPA, the training programme was harmonized in 2015 with three days refresher training and a five days training programmes. In 2016, UNFPA trained a pool of 15 trainers on

CMR/PFA who are involved in the implementation of the 2017 CMR/PFA training action plan.
To sustain and streamline current initiatives, it is important for the government of South Sudan (MoH) to develop context specific framework with clear guidelines to ensure systematic availability and accessibility of quality Health services to respond to sexual violence.
A consultant is therefore required to assist MoH to develop the Guideline/ Protocol based on the experiences outlined above and South Sudan context.
Scope of work:

To develop Service Delivery Guideline/ protocol on CMR Below are the main tasks:-
The overall Task of the consultancy is to develop the service delivery guideline/ protocol for CMR for the MoH. The following are the specific tasks:- Analyze existing health, legal and psychosocial support services available for survivors of sexual violence as well as existing CMR capacities/ initiatives at the health facilities in order to contextualize CMR service delivery protocol for South Sudan Assist/support in establishing/strengthening necessary tools if required for collecting information on CMR services Harmonize existing tools/modalities if any (e.g medical certificates) by giving relevant attention to the different systems and approaches by various CMR service providers in South Sudan including private and NGO managed Health Facilities Work closely with the Ministries of Health to lead consultation process with other Government line ministries (e.g. Ministry of Gender: Child and Social services: Ministry of Justice: forensic department: Police), civil society organizations and international organizations (particularly IRC, IMC, AMREF among many others) and UN Agencies (UNFPA, WHO, UNICEF, IOM) Contextualize, adapt and finalize CMR service delivery protocol to streamline and harmonise approach for South Sudan Conduct consultation and validation workshop with relevant partners
Duration and working schedule:
The total duration of the consultancy will be 45 working days (delivered in four Phased ) as follows:- Phase 1: 15 days ( consultations at Juba level (5 days), and selected States (10 Days) Phase 2: 15 days (home based) synthesis and write up of draft protocol
Phase 3: 5 days (Juba Based), preparation and presentation of Draft at Validation meeting. Phase 4: 10 days (home based) finalization and submission of the final CMR protocol to MoH and UNFPA
Place where services are to be delivered:
South Sudan (Juba) 10 days and 10 days in selected states
25 days will be home based
Delivery dates and how work will be delivered (e.g. electronic, hard copy etc.):
Delivery of the assignment within the 45 days (upon Signing of Contract)
Develop service delivery service delivery guideline/ protocol for CMR in South Sudan Develop a Capacity building plan for targeted Health facilities in rolling out the guideline Final consultancy report with clear recommendations for the implementation of the guideline/Protocol
Monitoring and progress control, including reporting requirements, periodicity format and deadline:
MoH and UNFPA will be Responsible to Monitor Consultant s work based on agreed Inception report and agreed deliverables. The final deliverables will be submitted to UNFPA on the 45thday, as the last day of the consultancy.
Supervisory arrangements: UNFPA Gender Specialist and MoH (RH Director) Expected travel: Within Juba (10 days and other states (10 days )
Phase 1 and 3: to and from Juba Phase 2 and 4 : no travel required
Required expertise, qualifications and competencies, including language requirements:
Academic Qualifications
University Degree in Medicine and Public Health 7- 10 Years’ experience in strategic work related to RH, GBV with a focus on CMR Proven understanding of the Survivor centred approach and guiding principles of working with survivors Field experience on CMR Knowledge and familiar with Medical protocols mainly STIs, HIV etc. Excellent interpersonal skills and ability to establish effective and working relationship with MoH, and other partners Good knowledge of South Sudan medical, Social, Health and Judicial context as well as of relevant I/NGOs and Public health
sector that might be relevant to the Clinical Management of Rape Forensic medicine experience is a plus. Excellent written and spoken Communication skills in English

Excellent knowledge of written and oral communication in English.
Computer skills: full command of Microsoft applications (word, excel, PowerPoint) and other relevant software based on the assignment.
